Fun, but it would have been better if it was harder and had a few more levels.

By level 4, I have already spawned in the wrong location and magically teleported out of the level 3 times and the only way to get back to gameplay is to refresh the game. It needs a lot of work.

It's not quite bad, if it's your first try in game design. Let me give you a few sugestions:
1st - Respawning. You die, you start again. Ever heard of lifes?
2nd - Graphic. Now really a must, but if you wanna have a good game around... If it's 8-bit-ish, better, people will think it's oldschool.
3rd - Coins have a use, or count as a real score.
4th - Bigger maps, that are labyrinths. The coins just gave away where I should go, like it wasn't already predictable.
5th - Maybe game modes, like scoring, racing against time, finding all the coins, stuff like that.
I hope this helps a little.
