Bugged my way into a cave wall and couldn't move afterwards.

Interesting bug: died and when I respawned the guy froze mid-air and his weapon kept changing.

It's also happened three times total.

So I go to do a 1-star mission, and suddenly I have to fight "The Apprentice" and he one-hits each member of my party. Why does this happen?
